Vizient Southern States offering enhanced collaboratives for Managing Back Pain and Spinal Surgery, and Diabetes Management
Open to Vizient Southern States' members, these enhanced collaboratives will address issues related to the implementation of the CMS’ Meaningful Measures for spinal surgery outcomes and the clinical impact of diabetes including complications, mortality and cost.
Both will connect members with their colleagues and subject matter experts to collaborate and learn leading practices, as well as identify areas for improvement.
The Managing Back Pain and Spinal Surgery Collaborative will have its kick-off meeting on April 25, 2018 and will address specific opportunities related to back pain management and spinal surgery across the continuum. These include:
- Rehabilitation and non-operative spine services
- Pre-admission care and selection
- Surgical care
- Post-operative inpatient care
- Transition of care and discharge planning
- Community partnerships and engagement
The Diabetes Management Collaborative will kick-off its first meeting on August 15, 2018 and will address specific opportunities related to diabetes management across the continuum. These include:
- Pre-diabetes screening and risk assessment in the community and at physician practices
- Self-management of risk factors associated with pre-diabetes and diabetes
- In-patient and out-patient diabetes education
- Glycemic control and diabetes management during hospitalization
- Transition of care and discharge planning
- Community partnerships and engagement
